Person-Centered Planning

Our approach is built around person-centered planning, where the individual is at the heart of all decisions. Unlike some organizations that offer both case management and direct services, we focus exclusively on case management, removing any potential conflicts of interest. This independence allows us to connect individuals and families to a variety of community resources, ensuring they receive the necessary supports in a timely manner. We coordinate and monitor services to guarantee that every individual’s needs are met.

Advocacy and Empowerment

We believe in knowing each individual deeply, understanding their unique goals, dreams, and needs. Instead of saying, “We can’t,” we ask, “How can we make this happen?” Our role is to advocate for those we serve, ensuring their voices are heard and respected in every decision.

Where We Provide Services

Our case management services are delivered within the community and at the individual’s residence. The case manager works closely with the individual, their guardian, and/or family to explore various service options. Together, the team identifies and coordinates the supports and services best suited to the individual’s needs and goals.
